Medications such as Ozempic, Wegovy and Mounjaro are becoming increasingly common. These GLP-1 receptor agonists are used to treat type 2 diabetes and support weight management, and for many people they offer significant benefits — including improved blood sugar control, weight loss, and reduced cardiovascular risk.

They have a high safety profile overall, and for many patients they are transformative. However, in this evolving landscape of modern metabolic treatments, it’s important to understand how they may occasionally affect eye health.

Why can GLP-1 medications affect vision?

The retina (the light-sensitive layer at the back of the eye) is very sensitive to changes in blood sugar levels.When blood sugar improves steadily, this reduces long-term risk of diabetic retinopathy. However, in some cases, rapid improvements in glucose levels can temporarily worsen existing diabetic eye disease. This is sometimes called “early worsening” and has also been seen with intensive insulin treatment.

It’s important to stress: the medication itself is not directly damaging the eye. Instead, sudden metabolic shifts can temporarily stress the tiny blood vessels in the retina.

Some people may also have undiagnosed diabetes or early retinal changes without knowing it, which is why regular eye examinations are so important — even if you feel your vision is fine.

What if you’re taking them for weight loss?

GLP-1 injections are also prescribed to patients without diagnosed diabetes.

There have been rare reports of non-arteritic anterior ischaemic optic neuropathy (NAION) — a condition caused by reduced blood flow to the optic nerve. A direct causal link is still being studied, and cases are uncommon, but sudden vision loss should always be treated as urgent.

Again, these events are rare, and the overall safety profile of these medications remains strong.

Symptoms you should never ignore

  • Dark shadows or missing areas in your vision
  • Distortion (straight lines appearing wavy)
  • Sudden vision loss in one eye
